Verification Statement and Final Transcript

All incoming dietetic interns to the Sodexo Dietetic Internship Program will be required to submit the following documentation prior to the first day of intern orientation:

• Verification Statement from their DPD Program including date of completion (month, day, year) and original signature of the DPD director in non-black ink. The DPD program director must sign the form on or after the completion date of the requirements.

• Final transcript from their university indicating that the degree has been conferred.

Upon receipt of this documentation, the dietetic internship director will review the forms to verify completeness and accuracy. Any discrepancies identified with the documentation will be resolved by the dietetic internship director, intern and DPD director. Interns will not be allowed to begin orientation without these documents in their file.
