COVER LETTER.  Include a cover letter or note with your application telling us what geographic location(s) you are applying for--otherwise we will assume you are willing to be placed anywhere.  If you include your e-mail address in your cover letter, we will notify you when we receive your application and let you know if there are any items missing.

APPLICATION.  Use the ADA APPLICATION FORM.  The new ADA application form only asks for dietetics-related experience in the last 5 years.  We are interested in all of your experiences whether they are related to dietetics or not.  Either add all jobs into the application form, or send a resume or separate list of your jobs and volunteer experiences.  Include you major tasks/responsibilities, dates you worked and the number of hours you worked.  If you have significant work experience prior to the last 5 years, feel free to include that as well.

THREE (3) RECOMMENDATIONS (include one from an instructor in a major subject and at least one from a work supervisor--NOTE:  These are both required even if you are currently not in school).  Be sure to have the recommendations sent sealed with the writer's signature across the flap of the envelope. USE ONLY THE SODEXO RECOMMENDATION FORM INCLUDED ON THIS WEB PAGE.

DPD VERIFICATION STATEMENT OR DECLARATION OF INTENT (This must be signed by the official Program Representative.  Signature must be original and in a color other than black ink).  PLEASE NOTE:   Plan IV Verification Statements are no longer valid.  Students must have a DPD Verification Statement prior to starting the program.  Applicants who have a Plan IV verification should contact their DPD director to discuss requirements for obtaining a DPD Verification prior to starting the Internship.
