Application Form - Standard ADA form for internship programs.  Grade point average must be calculated as directed.  Grades and GPA’s will be verified in the screening process. Photocopies are not acceptable.  Please note that although the application calls for GRE scores, they are not required.  Click Here for a copy of the ADA application

Application Fee – The application form must be completed entirely and returned along with a non-refundable check or money order payable to: Sodexo.  Application fee is $50 to apply for one emphasis (general or pediatric) and $75 to apply for both emphases.

Official Transcripts - Complete through the first semester senior year from all colleges and universities attended or through graduation if you have graduated.  Final official transcripts will be required subsequent to appointment for remaining courses included on the Declaration of Intent Statement.  Photocopies are not acceptable.  Transcripts must be in sealed envelopes that bear the signature of the registrar over the seal.  Click here to view the Verification and Transcript Policy.

ADA Verification Statement - or Declaration of Intent Statement for the Didactic Program in Dietetics completed and signed by your Didactic Program Director. Photocopies are not acceptable.  (Verification Statement will be required prior to beginning program.)

Three Letters of Reference - These should provide pertinent information as to your scholastic abilities, prior work experience, potential professional qualifications, character and personality.  They should be written by professors in your major and work supervisors in the field.  Letters of reference may be included in the packet if they are in sealed envelopes that bear the signature of the person providing the reference over the seal.  The recommendation forms provided with the ADA Application should be used for this requirement.

Handwritten Letter of Application - Stating your background, interests, goals and identifying skills that you have developed and work experiences which make you a desirable candidate.

COMPLETED PACKET SHOULD CONTAIN:

• Official transcripts from all colleges or universities attended, or include a note that indicates the colleges or universities that will mail transcripts directly to the program director.
